Candidate Questionnaire for the 2005 Voter Guide

Instructions:

The questionnaire is available as a Word document or a text file. Download either version by right-clicking the link and selecting 'save target as...'   
Then select a folder on your computer in which to save the file and click 'Save'.

1.  Complete the questionnaire and email to votersguide2005@lwvcef.org

2.  Then print the completed questionnaire, sign it, and do one of the following:

Please note we are only able to accept submissions via email. We must have candidate responses no later than October 1 in order to be published in the online guide. You may also email a photo. If you have a preferred mailing address, please note it on the questionnaire when you return it to us.
